Care Coordinator - RN - Health-8715
Interesting opportunity with a startup backed by large insurance company that focuses on improving Kidney Disease in America by patient education, preventative care, patient advocacy and disease management. This particular role is a clinical role/Care Coordinator and needs to be based in North Carolina and will require travel within the state at in-home/place of convenience visits with kidney disease patients. No office as this role will be remote/work from home arrangement. This role will be responsible for implementing client's clinical and operational programs designed to improve patient outcomes and affordability.
Collaborates with nephrology practices to identify members, and to develop a patient-specific education plan based on the physician's plan of care.
Actively partners with members diagnosed with chronic kidney disease to: provide education and support relative to the management of their disease and treatment options, develop healthcare goals and identify potential barriers to achieving those goals, identify member support systems, and collaborate to create solutions to overcome barriers.
Utilizes motivational interviewing techniques to help members and caregivers identify and understand their healthcare goals, and motivate members to engage in positive behavior change.
Initiates and drives data collection and reporting processes that provide transparency on the execution against operational KPIs.
Participates with local leadership in executing on operating strategies required to meet the company's overall mission and goals.
Identifies process improvement needs and participates in the development of new and improved clinical programs.
Current state RN license
5+ years of experience in nephrology nursing
2+ years of experience in dialysis home modalities
1+ year of experience in ESRD/CKD education
Skills that would be a plus include:
Prior or current experience in home health
Strong interpersonal and communication skills
Bilingual in English/Spanish
Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
